Risk Assessment Case Study 1

Location: Three Mills Studios STAGE 11
Brief: To build a 13' by 35' scaffold frame clad in 3/4 8x4 plywood with 3x2 framework. Overall height 13' Studio - To be anchored in place by use of bracing.

Hazard Identified: Men working at a height
Severity: High
At Risk: Riggers
Precautions: Riggers to wear safety harnesses, and relevant safety equipment

Hazard Identified: Bad lifting of all equipment Severity: Low
Likelihood: Low
At Risk: Riggers Precautions: All Riggers are experienced in lifting the equipment with which they will be working. Riggers to wear steel capped protective shoes and protective headgear.

Hazard Identified: Contact with overhead lights of wires
Severity: High
Likelihood: Low
At Risk: Riggers
Precautions:.There are no apparent visible electrical wires or lights in the working area.

Hazard Identified: Objects falling from height
Severity: High
Likelihood: Low
At Risk: Production personnel and crew
Precautions: Area immediately below will be kept clear of all unnecessary personnel and crew until entry is authorized by the Charge-hand Riggthe tower. Qualified riggers will be used to rig and brace.

It is the policy of Blitz Rigging to develop a positive health and safety culture throughout the organisation because we believe that high standards of health and safety are necessary for company efficiency and competitiveness.

To achieve this the company will continually strive to identify all workplace hazards and take appropriate measures to eliminate or control risks to employees and others affected by our operations by applying positive control standards and provision of information, training and supervision as needed.

Employees at all levels are reminded that they have a duty, commensurate with their position, to ensure that the company's health and safety policy is observed. In particular they are required:-

To take reasonable care for their own health and safety at work and of those who may be affected by their actions, or by their omissions.

To co-operate with their employer to ensure that any duty, or requirement, for health and safety imposed upon their employer by law is performed or complied with.

To co-operate with any other holder of health and safety duties (such as contractors or other employers working at our premises, and other employers when you are working at their workplaces) as far as is necessary to enable them to perform their duties.

Not to intentionally, or recklessly, interfere with or misuse anything provided in the interests of health, safety or welfare.

To report to supervisory staff hazardous conditions or defects in the company safety arrangements.

The company recognises that it has the ultimate legal responsibility for health and safety. Accordingly the Managing Director accepts overall responsibility for policy formulation and implementation. In turn all levels of management and all supervisors are responsible for carrying out those health and safety duties placed upon them. The Managing Director will ensure that the safety plan includes sufficient resources for the successful implementation of the Health and Safety Management Policy.

Development of the Health and Safety Management Policy will be conducted through normal business meetings where health and safety will be given standing equal to other activities of the business.
The company will appoint a number of competent persons to assist the company directors in implementing this policy. The Safety Officer's role is to provide independent and authoritative advice to managers with individual responsibilities for health and safety.

All our Experienced Riggers are BECTU registered and have undergone the following minimum training:-

Scafold Safety Inspectors

Sample Method Statement


Contractor: Hermes GB / Fiona Rushton
Site: New Bond Street, London (To take place 17/10/09 at 05:00 AM)
A risk assessment will be carried out by the supervisor in order to minimise risk to the
Health & Safety of our employees, those using the scaffold tower and any other persons affected by our work. This will be amended or updated throughout the job as necessary. Material will be inspected before loading in a safe manner that does not present risks in the transportation. The material will be stacked on site so as to minimise the risk of displacement, or forming a tripping hazard.

The labour used will be competent persons and will be instructed by the supervisor who is directly responsible for all aspects of the job on site. The instruction will include provision of a quotation, answering any queries that may arise from the quotation, contract or working environment. Our operatives will be made aware of any conditions imposed upon us by the customer or working environment. Operatives will be issued with, and will wear/ use the appropriate personal protective equipment. All personnel will report to the appropriate person each time they attend and leave site. They will obey site rules and display the necessary identification badges where necessary.

The scaffold tower will be erected with minimum risk and generally in accordance with the quotation. Major variations to the quotation will only be made with the prior approval of the supervisor or more senior member of staff. Material will be raised and lowered by hand or rope and wheel where necessary otherwise by hand with rope. All work carried out will be within C I T B guidelines for erection and dismantling of scaffold tower. Our operatives are issued with harness', and will follow the guidelines for the use of fall arrest equipment.
The supervisor will check the scaffold tower during erection as necessary and always on completion.
The above sequence of events will be carried out in reverse for the dismantling.
